PARTICIPANT ACCIDENTAL MEDICAL POLICY

BELOW THOROUGHLY

One of
the advantages of being an ASA Member Track is that you the racers,
drivers & crew have the opportunity to apply for $500,000.00
Participant Accidental Medical Policy if
you are to be injured at HMP.
The price is very minimal considering medical costs as they are in
this day and age.
Also included with the ASA Policy:
ASA Baseball Cap
ASA Hard Card - License
ASA Decal
The
Circle Track / Stock Car Racing Magazine Subscription
ASA Patch - For PRO LATE MODEL DRIVES ONLY (To Go On Driving Suit)

Late Models - MANDATORY TO RACE AT HMP

You will not be eligible to race at HMP.

Owner - $125.00 For Season (Works Out To Be $7.82 Per Race Meeting)
Driver - $125.00 For Season (Works Out To Be $7.82 Per Race Meeting)
Crew - $85.00 For Season (Works Out To Be $5.31 Per Race Meeting)

All
forms must be mailed by February 12th to ASA as it takes 30 days
for Medical Coverage Policy to take effect.
The Application MUST to filled out entirely.
Both the front and back must be signed and dated.
If any information is missing your application will be mailed back to
you to be completed and will not be active until is received and COMPLETE.
These forms are available at the Track or you can request
I mail an application(s) to you.
If you would like an application mailed to you, please send me an e-mail
which includes you Name, Address, and how many forms you would
like mailed to you.
Please title the e-mail ASA Application.
You can also turn in the form to me (SARAH)
at the track and I can verify it is filled out correctly (saves any
chance of it not being correct) and can mail it with the other applications
I have received.
